Travel Physical Therapist - Home Health

American Traveler is seeking a Home Health Physical Therapist with 2+ years of experience and current Montana PT licensure to provide skilled therapy services to patients in their homes. Requirements Current PT license in the state of Montana required, if applicable Minimum 2 years of experience required BLS certification required Valid driver's license required Responsibilities Home Health and Hospice setting 36 hours per week, day shift 13-week contract Travel to patient homes and other care settings to provide therapy services Receive and review physician therapy orders for Home Health and Hospice patients Complete comprehensive patient evaluations and establish written treatment plans with short-term and long-term goals Provide skilled interventions including therapeutic exercises, stretching, mobility training, balance activities, pain management techniques, and functional rehabilitation Assess home safety and recommend equipment, assistive devices, or environmental modifications to promote independence and prevent falls Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes Educate patients, caregivers, and family members on treatment plans, home exercise programs, safety measures, and expected outcomes Maintain accurate and timely documentation including evaluations, progress notes, discharge summaries, recertifications, and resumptions of care Collaborate and communicate with physicians, nurses, therapists, and other healthcare professionals involved in patient care No holiday coverage required Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Job ID: P-763677 Open Positions: 1 #J-18808-Ljbffr American Traveler, LLC.
