Key Responsibilities

• Conduct thorough evaluations of patients in their homes to assess their physical condition and establish effective treatment plans tailored to their specific needs.
• Utilize a variety of therapeutic techniques such as therapeutic exercises, manual therapy, and innovative modalities to reduce pain and enhance mobility.
• Collaborate closely with patients, caregivers, and other healthcare professionals to ensure continuity of care and optimal recovery outcomes.
• Document treatment sessions accurately, tracking patient progress rigorously to inform ongoing adjustments to therapy plans.
• Engage in ongoing re-evaluation of patients' progress and modify treatment strategies to challenge and support their recovery journey.
• Recommend and fit assistive devices to enhance patients' self-care and mobility, ensuring they have the necessary tools for success.
• Provide comprehensive training and education for patients, families, and caregivers on the use of therapeutic techniques and equipment.
• Stay up to date with advancements in physical therapy practices by attending workshops, participating in professional organizations, and pursuing ongoing education opportunities.

Patient Population / Client Focus

Your primary focus will be on a diverse patient population, including children and adults dealing with various injuries or conditions. You will work with individuals recovering from orthopedic surgeries, managing chronic pain, or rehabilitating after sports injuries. Each patient requires a personalized approach to therapy, where you play a crucial role in facilitating their path to recovery.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required for this role. Candidates should hold a valid state license in Physical Therapy to practice in Florida.
• Strong communication skills, both written and verbal, allowing for effective interaction with patients, families, and other healthcare professionals.
• Computer proficiency for documentation and communication purposes within our electronic health record (EHR) systems is necessary.
• A flexible and agile approach to patient care is essential, accommodating the dynamic needs of home health therapy.
• While prior experience is valued, motivated new graduates are encouraged to apply, with mentorship opportunities available for those just entering the field.

Preferred Experience

A minimum of two years' experience as a physical therapist is preferred, ensuring familiarity with best practices and clinical techniques that enhance patient outcomes. Experience in a home care environment will be considered an asset.
