This full-time position is primarily dedicated to supporting patients at our detox center, where you will play a crucial role in facilitating recovery and ensuring a smooth transition for individuals grappling with substance use disorders. Given the unique nature of substance use recovery, this role involves direct interaction with a variety of individuals from different backgrounds. As part of this dedicated team, you will engage in ongoing clinical discussions, participate in program outreach, and maintain an active presence in facilitating therapeutic sessions.

Key Responsibilities\n

• Conduct thorough clinical evaluations of patients upon admission, assessing for both individual and group therapy needs.

• Lead psychoeducational and process-oriented therapy groups designed to empower patients and aid in their recovery journey.

• Ensure a seamless discharge planning process starting on the day of admission by confirming appointments with patients and their families.
\n
• Actively communicate with referral sources and outpatient providers to maintain continuity of care throughout the patient’s treatment.
\n
• Facilitate patient transfers to higher levels of care as required, ensuring patients receive the necessary support.
\n
• Develop and implement SMART treatment plans tailored to the individual needs of each patient.
\n
• Manage crises as they arise during therapy sessions and within the treatment milieu, providing immediate interventions and support.
\n
• Collaborate with the entire treatment team to ensure a high level of clinical quality and support.
\n
• Participate in ongoing clinical outreach and represent the organization in program tours as needed.
\n
• Provide clinical support to other team members when necessary, ensuring comprehensive treatment for all patients.

Patient Population / Client Focus\n

In this role, you will work predominantly with individuals experiencing substance abuse and behavioral health issues. Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or another related mental health discipline.

• Must hold a current and valid Licensed Clinical Social Worker (LCSW) credential and be independently licensed.

• Proven experience in a clinical setting, particularly in substance use treatment and mental health counseling.

• Strong familiarity with electronic health record systems and a proficiency in Office 365.
