Key Responsibilities\n

• Conduct comprehensive mental health assessments tailored to individual patient needs to develop detailed and personalized treatment plans.

• Deliver evidence-based therapeutic interventions to individuals, couples, and families, utilizing techniques such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care.

• Facilitate psychoeducational and support groups that help patients and families address common mental health concerns and establish a sense of community.

• Collaborate closely with psychiatrists, psychologists, and multidisciplinary team members to ensure cohesive and holistic patient care.

• Provide crisis intervention services, offering immediate support during mental health emergencies.

• Educate patients and their families about mental health conditions, treatment options, and available community resources to foster understanding and empowerment.

• Maintain accurate, confidential documentation in accordance with all legal and ethical standards using electronic health record (EHR) systems.

• Participate actively in case consultations and contribute to the professional development of the team.

Patient Population / Client Focus\n

This position primarily serves clients in a hospice setting, focusing on individuals who may be experiencing severe mental health challenges at the end of life. Qualifications\n

• A Master’s or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution is required.

• Must possess current and valid licensure as an LCSW, LPC, or LMFT in the state of Arizona.

• Demonstrated knowledge of mental health disorders and evidence-based treatment modalities, along with a proven ability to conduct thorough mental health assessments.

• Experience providing individual and group therapy to diverse populations, including special focus on geriatric and hospice clients.

• Strong interpersonal communication skills, with the ability to establish rapport and trust with clients and colleagues alike.

• Exemplary judgment, adherence to professional ethics, and the capacity to uphold boundaries with clients.

• Familiarity with electronic health record (EHR) systems and the ability to document effectively.

• A collaborative spirit and the willingness to work as part of a multidisciplinary team.

Preferred Experience\n

• Minimum of 2 years of experience in a clinical mental health setting is preferred, ideally with hospice or end-of-life care experience.

Compensation and Benefits\n
