Key Responsibilities\n

• Conduct triage for both urgent and routine referrals received from medical professionals, ensuring timely responses to critical mental health needs.

• Collaborate with a dedicated team of healthcare providers to schedule initial intake appointments, minimizing delays in patient care.

• Engage actively in care team huddles that occur within the primary care departments, contributing insights based on your therapeutic expertise.

• Develop and maintain client-centered treatment plans tailored to the individual needs and goals of your clients, utilizing evidence-based methodologies.

• Handle case management tasks, providing advocacy and coordination of services, including medication monitoring and access to emergency services when necessary.

• Collaborate with insurance companies and other agencies for seamless patient support, ensuring that clients have access to the resources they need.

Patient Population / Client Focus\n

You will work with a diverse population, including adults, couples, and families facing various challenges, from anxiety and depression to relationship issues and trauma. Qualifications\n

• A Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling is required.

• Current, active licensure in the State of Pennsylvania as an LMFT, LPC, or LCSW.

• Self-directed and self-motivated, demonstrating initiative in a remote working context.

Preferred Experience\n

• Previous telehealth experience is preferred but not mandatory; however, a comfort level with digital communication platforms is necessary.

Compensation and Benefits\n

The salary range for this position is competitive, offering between USD 90,000 and 100,000 annually, based on your caseload and experience level.
