Key Responsibilities\n

• Conduct thorough assessments, including diagnosis when necessary, to evaluate patients’ mental health status and effectiveness of care. Utilize tools such as the Diagnostic and Statistical Manual of Mental Disorders to create individualized treatment plans.

• Continuously monitor patient progress and provide evidence-based interventions and support to align with mental health diagnoses.

• Ensure that all treatment records, reports, and clinical information is meticulously prepared and maintained, adhering to regulatory requirements and best practices in documentation.

• Collaborate with patients and their families to formulate comprehensive treatment plans, addressing individual needs and desired outcomes.

• Develop treatment goals in conjunction with patients and their families, actively intervene in areas of patient safety, and symptom management, and work within a collaborative team framework.

• Coordinate referrals and act as a vital link between patients and other treatment providers, building and maintaining professional relationships with clinicians and referring agencies.

• Engage in mentoring and educational activities for students, interns, residents, and volunteers, enhancing their development and understanding of clinical practices.

• Participate in peer review, clinical collaboration, and chart auditing under the guidance of your clinical supervisor, contributing to the professional growth and development of your colleagues.
