Key Responsibilities\n

As a Licensed Clinical Social Worker, you will report directly to the Social Work Manager and take on a variety of responsibilities, including:

• Conducting comprehensive bio-psychosocial evaluations to identify barriers to treatment and enhance patients' health statuses.

• Developing individualized treatment plans that incorporate evidence-based interventions tailored to the specific needs of patients.

• Implementing therapeutic interventions in both individual and group settings to improve patient outcomes.

• Collaborating closely with medical and support staff to provide integrated care, ensuring a seamless experience for patients.

• Documenting all social work activities in patients' medical records in accordance with departmental standards and regulations.

• Offering education to patients and their families to empower them to understand and adhere to medical and social work recommendations.

• Assessing advocacy needs for patients and families, coordinating with community resources to enhance social support.

• Staying updated on available community resources relevant to patient care and accessing them as needed.

Patient Population / Client Focus\n

Your role will primarily focus on patients attending our hospice center who are facing complex psychosocial challenges due to chronic illness or terminal diagnoses. You will provide tailored support to diverse patient populations, applying your expertise to address their unique needs across various life stages and cultural contexts.

Qualifications\n

The ideal candidate will possess:

• A Master’s Degree in Social Work from an accredited institution.

• A current Licensed Clinical Social Worker (LCSW) license issued by the State Board of Social Work Licensure.

• A strong understanding of ethical standards in social work practice based on the NASW Code of Ethics.

• Exceptional communication and analytical skills necessary for evaluating patients and developing effective treatment strategies.

Preferred Experience\n

While a minimum of six months to one year of clinical experience is required, we highly prefer candidates with:

Compensation and Benefits\n

This position offers a competitive salary range of USD 85,000 - 100,000 per year, commensurate with experience.
