Key Responsibilities\n

As a Licensed Professional Counselor, your responsibilities will include:

• Conducting comprehensive assessments and ongoing evaluations to identify the mental health needs of patients.

• Maintaining effective communication with supervisors, family members, and other care providers to report on patient progress and coordinate care.

• Organizing and scheduling follow-up appointments, home visits for observation, and data collection to stay informed on patient advancements.

• Ensuring that individualized treatment plans are implemented effectively and in accordance with best practices.

• Participating in Interdisciplinary Team Meetings to foster collaboration and holistic treatment planning.

• Documenting patient progress and outcomes through monthly and quarterly reports, ensuring compliance with legal and ethical standards.

• Training staff members on the implementation of program plans and necessary documentation techniques.

• Providing ongoing therapeutic support through follow-up interviews and personalized home visits.

• Acting as a liaison within the mental health community to enhance access to services and promote program goals.

• Developing procedures and training materials to continually improve the quality of programs we offer to our clients.

Patient Population / Client Focus\n

Your role will focus on providing counseling services to a broad spectrum of individuals, including both adults and youth. Qualifications\n

To be considered for this role, candidates must possess:

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.
