Work Environment\n

This role is designed to be fully remote, offering you the flexibility to manage your work schedule effectively. Your typical hours will be from Monday to Friday, 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours based on client needs. The position provides an excellent work-life balance, while our commitment to team communication ensures you are well-supported by staff through regular virtual meetings and collaborative tools. On-call availability may be required to address urgent situations for our clients.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for seniors, utilizing assessment tools and interviewing techniques to evaluate emotional, mental, and social factors impacting their lives.

• Collaborate with clients and their families to create and implement personalized care plans that outline specific objectives and preferences, ensuring their unique needs are met.
\n
• Provide emotional support through counseling sessions, assisting clients and their families in managing life transitions such as loss, illness, or changes in living situations.
\n
• Advocate for clients by connecting them with essential community resources, services, and support networks that enhance their quality of life.
\n
• Work in collaboration with interdisciplinary teams of healthcare professionals, including nurses and caregivers, to provide cohesive and comprehensive care to clients.

• Conduct regular follow-up visits via video calls to monitor clients' well-being, evaluate their home environments, and address safety issues or additional concerns.

• Facilitate effective communication among family members regarding care issues and develop mediation strategies to resolve conflicts.
\n
• Educate seniors and their families on available community resources and programs that encourage social engagement and overall wellness.
\n
• Maintain accurate client records, including assessments, care plans, and progress notes, ensuring compliance with state regulations and ethical standards.

Patient Population / Client Focus\n

You will primarily be working with seniors, many of whom may have complex health conditions or face psychosocial challenges. Your role will focus on understanding their emotional needs, developing treatment plans, and working closely with family members. By addressing not only the clinical aspects of care but also the emotional support required, you will significantly enhance their overall well-being.

Qualifications\n

• A valid and current state licensure as a Licensed Clinical Social Worker (LCSW) is essential for this role.

• A bachelor's degree in Social Work; additional qualifications, such as a master’s degree in a relevant field, are preferred.

• A minimum of two years of experience in a social work role, particularly in geriatric care, community settings, or similar environments.

• Extensive knowledge of social work principles, especially as they pertain to aging and available community resources.

• Exceptional assessment, counseling, and problem-solving skills, with a focus on empathy and compassion.

• Strong ability to collaborate within interdisciplinary teams and communicate effectively with clients and their families.

• Familiarity with electronic health record (EHR) systems and standard computer applications.

Preferred Experience\n

Compensation and Benefits\n

The salary for this position ranges from USD 60,000 to 100,000 per year, commensurate with experience and qualifications.
