Key Responsibilities\n

Your day-to-day duties will include a range of responsibilities designed to provide exceptional care:

• Conduct initial and ongoing assessments to identify patient needs, preferences, and therapeutic goals, ensuring a personalized treatment plan.

• Facilitate effective communication among supervisors, healthcare team members, family members, and external resources to monitor and discuss patient progress.

• Maintain a structured schedule of patient appointments, home visits, and therapeutic observations to ensure progress and program adherence.

• Plan and coordinate home visits, performing in-home assessments and collaborating with other healthcare professionals to evaluate and adjust treatment strategies.

• Implement treatment plans and monitor their effectiveness, making necessary adjustments as needed to enhance patient outcomes.

• Participate in interdisciplinary team meetings to discuss cases, share insights, and foster teamwork across various healthcare specialties.

• Document client progress through monthly and quarterly reports, tracking improvements and adjusting care plans accordingly.

• Organize training sessions to ensure clinical staff are well-versed in program implementation and documentation protocols.

• Provide follow-up support through interviews and additional home visits to address evolving patient needs.

Qualifications\n

To be successful in this role, you will need the following qualifications:

• A Master’s or Doctorate degree in a relevant field (Counseling, Social Work, Psychology, etc.) is required.

• Active and unrestricted licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW) in California.

• A minimum of 2 years of experience in counseling, preferably within a clinical setting, demonstrating your capability to handle a diverse caseload.

• A strong specialization in areas such as family therapy, couples therapy, and trauma-informed care is preferred.

• Robust understanding of legal, ethical standards, and best practices in mental health counseling.

• Proven ability to work collaboratively within a healthcare team.
