Key Responsibilities\n

• Engage clients in one-on-one sessions to assess their mental health status, discussing their goals, preferences, and treatment options before formulating individualized rehabilitation plans.

• Develop and continually adjust rehabilitation plans that are aligned with clients' educational backgrounds, skills, physical capacities, and future employment ambitions.

• Collaborate closely with a team of healthcare providers to create and implement integrated treatment programs tailored to each client's unique circumstances and needs.

• Maintain accurate and up-to-date case files and records, documenting personal information, insurance details, services rendered, and client interactions with attention to detail and confidentiality.

• Monitor client progress diligently, documenting advancements and setbacks to determine the effectiveness of treatments and make necessary adjustments.
\n
• Establish ongoing communication with clients during their participation in job training and occupational placement programs, promptly addressing any issues that arise.

• Support clients by liaising with their family members to develop effective strategies for meeting identified needs and enhancing overall support systems.

• Perform thorough needs analyses for clients and compile comprehensive reports that inform treatment and resource recommendations.

Qualifications\n

• A Master’s degree in a relevant field, such as counseling, social work, or psychology.

• Must possess a valid Licensed Professional Counselor (LPC) licensure in Connecticut.

Compensation and Benefits\n

As a valued member of our team, you will receive an annual salary ranging from USD 85,000 to 100,000 , commensurate with your experience and qualifications. Additional benefits include:
