Key Responsibilities\n

• Develop comprehensive service plans tailored to the individual goals of clients, incorporating interventions and progress monitoring to ensure effective treatment.

• Provide expert counseling and emotional support to individuals and families facing transitional life challenges associated with hospice care.

• Collaborate with clients to create personalized service plans, facilitating access to community resources such as healthcare services, financial assistance, and support groups.

• Implement crisis intervention strategies, including risk assessment and safety planning, to support clients experiencing acute emotional distress.

• Engage in outreach activities to raise awareness of the agency's services, benefitting both clients and the broader community.

• Participate actively in case conferences and team meetings, sharing insights and strategies to enhance client outcomes.

Patient Population / Client Focus\n

This role primarily focuses on clients within the hospice program, a sensitive area of social work where the need for empathy, communication, and informed advocacy is paramount. Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited university.

Compensation and Benefits\n
