Work Environment

This full-time position is conducted onsite, with therapists providing in-home care across the Woodbridge area. Our therapists function as part of an interdisciplinary team, collaborating with healthcare professionals to optimize patient care. Key Responsibilities

• Develop and implement personalized physical therapy treatment plans aimed at preventing disability and relieving pain, while restoring muscle function according to each patient's capabilities.
• Supervise and guide Physical Therapy Assistants in the implementation of care plans, ensuring compliance and effectiveness in treatment delivery.
• Conduct thorough evaluations of each patient's physical function, utilizing diagnostic tests that assess muscular, neurological, and joint performance.
• Assess the patient's home environment, making recommendations for adaptive equipment or modifications to enhance independence and safety.
• Provide skilled physical therapy services in accordance with physician prescriptions, which may include manual therapy techniques and functional exercise programs.
• Continue to evaluate and document patient progress, adjusting treatment plans as necessary, and engaging with doctors and case managers concerning patient outcomes.
• Create and maintain detailed clinical records, including evaluations, treatment notes, and discharge summaries, ensuring accurate documentation for compliance with legal and professional standards.
• Participate actively in interdisciplinary team meetings to discuss patient care updates and strategize on holistic treatment approaches.
• Educate clients and their families on their roles in the therapeutic process, ensuring they understand treatment methods and care for assistive devices.
• Complete other duties as assigned, aiming to meet the organizational goals and deliver comprehensive patient care.

Compensation and Benefits

This position offers a competitive salary range of USD 100,000 – 120,000 per year, depending on clinical experience and qualifications.
