Work Environment\n

This position is fully remote, allowing you to work from the comfort of your home while maintaining a structured schedule. Our hours are Monday to Friday from 8:00 AM to 4:30 PM, although occasional evening or weekend hours may be required based on client needs. You will have flexibility in your scheduling to ensure consistent and personalized care for your clients while occasionally being on-call to address urgent situations.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, taking into account emotional, mental, and social dimensions to develop effective intervention strategies.

• Collaborate with clients and their families to create personalized care plans that reflect their unique preferences and goals.
\n
• Provide counseling and emotional support, assisting clients in navigating transitions such as health declines or loss, and addressing anxiety and depression.
\n
• Actively advocate for clients’ rights and facilitate access to essential community resources, ensuring they receive appropriate services and assistance.
\n
• Develop collaborative relationships with an interdisciplinary team of healthcare professionals, such as nurses and caregivers, to optimize the care and comfort of clients.
\n
• Make regular visits to clients’ homes or hospice facilities to monitor their well-being, assess living conditions, and address any potential safety concerns.
\n
• Facilitate effective communication among family members to address conflicts and enhance cooperative care for the senior client.
\n
• Educate clients and their families about local community resources, support groups, and programs that encourage social engagement and improve quality of life.
\n
• Maintain thorough documentation of client assessments, care plans, progress notes, and relevant interactions in compliance with ethical and regulatory standards.

• Stay current on trends in social work practices and engage in ongoing professional development activities.

Patient Population / Client Focus\n

This role primarily focuses on seniors within a hospice context, addressing the unique psychosocial challenges they and their families face during critical life transitions. You will work to empower clients by equipping them with coping strategies and resources, fostering a sense of control and dignity in their lives.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
\n
• Current and valid Texas state licensure as a Licensed Clinical Social Worker (LCSW).
\n
• Minimum of two years of experience in social work, preferably in a community-based or geriatric setting, with a focus on hospice care.

• In-depth understanding of social work principles, aging-related issues, and available community resources.
\n
• Exceptional assessment, counseling, and problem-solving abilities.
\n
• Strong interpersonal skills to build trusting relationships with clients and families.
\n
• Proficiency in electronic health record (EHR) systems and other relevant technology.

Preferred Experience\n

While at least two years of related experience is required, candidates with experience in end-of-life care, palliative care, or a hospice organization will have a distinct advantage. Familiarity with grief counseling and end-of-life issues is also highly desirable.

Compensation and Benefits\n
