This remote role allows flexibility while maintaining a supportive climate through digital collaboration tools. Although the majority of interactions will be conducted virtually, you will engage with a diverse group of healthcare professionals committed to multidisciplinary teamwork. The climate-controlled office environment ensures comfort during meetings that may occur on-site at our Hospice Center when necessary.

Key Responsibilities\n

• Conduct comprehensive assessments and utilize the Diagnostic and Statistical Manual of Mental Disorders to identify patients' mental health needs, crafting individualized care plans that consider diverse backgrounds and specific requirements.

• Monitor and evaluate patient progress continuously, implementing therapeutic strategies to effectively address mental health diagnoses and ensuring that treatment objectives are met.

• Maintain meticulous patient records, reports, and clinical documentation while adhering to state regulations and organizational standards using current technology.

• Facilitate collaborative development of comprehensive treatment plans with patients and their families, taking into account their goals and preferences to foster engagement in their care journey.

• Coordinate patient referrals to additional services, serving as an essential liaison between patients and other treatment providers to enhance care continuity.
