Key Responsibilities\n

In this role, you will:

• Conduct individual and group therapy sessions through secure video conferencing platforms, focusing on mental health support for patients and their families.

• Develop and implement personalized treatment plans that are tailored to each client’s unique needs and circumstances, utilizing evidenced-based practices.

• Collaborate with a multidisciplinary team including nurses, physicians, and other healthcare professionals to provide a holistic approach to patient care.

• Maintain accurate and up-to-date client records in compliance with HIPAA regulations and organizational standards, ensuring confidentiality and security.

• Engage in ongoing assessments and adjustments to treatment plans based on client feedback and clinical outcomes to optimize therapy effectiveness.

Patient Population / Client Focus\n

Your primary focus will be on clients in hospice care, many of whom are navigating complex emotional landscapes as they face end-of-life issues. You will work with diverse populations and address a wide range of mental health concerns, including depression, anxiety, grief, and adjustment disorders. Your role includes providing psychosocial support to not only the patients but also to their families, ensuring a compassionate and supportive approach.

Qualifications\n

The ideal candidate will possess:

• A valid Licensed Clinical Social Worker (LCSW) licensure in New York.

• A minimum of two (2) years of experience in a clinical social work setting, preferably with hospice or palliative care.

Compensation and Benefits\n
