Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of patients and their families, identifying specific concerns, strengths, and needs that inform treatment planning.

• Develop and regularly revise personalized treatment plans tailored to individual patient profiles, which may include both clinical issues and considerations of spiritual and cultural values.

• Facilitate educational and therapeutic groups to enhance patient coping strategies, and document observed patient goals and responses in accordance with treatment objectives.

• Participate in treatment planning meetings, providing and receiving insightful feedback related to patient progress, challenges, and evolving needs.

• Maintain up-to-date and accurate documentation of clinical services provided, ensuring compliance with hospice regulations and facilitating seamless continuity of care.

Qualifications\n

Compensation and Benefits\n

The annual salary for this position ranges from $120,000 to $130,000 , commensurate with experience and caseload adjustments.
