Key Responsibilities\n

• Provide individual and family counseling, focusing on adjustment issues related to rehabilitation, grief, and coping mechanisms, to enhance the patients' emotional well-being.

• Collaborate with a multidisciplinary rehabilitation team to develop tailored treatment plans that align with each patient's psychosocial needs and rehabilitation goals.

• Assist patients in navigating and accessing community resources, such as vocational rehabilitation programs, support groups, and housing assistance—as part of a holistic approach to recovery.

• Deliver education and ongoing support to patients and their families regarding available community resources and post-rehabilitation services to ensure continuity of care.

• Play a pivotal role in discharge planning, facilitating smooth transitions to home or other care environments, and ensuring patients' ongoing support systems are established.
