Key Responsibilities

• Develop and implement individualized treatment plans based on patients' specific conditions, goals, and progress.
• Guide patients through therapeutic exercises, tailored stretches, and functional activities designed to enhance mobility, strength, and overall physical well-being.
• Educate clients and their families on proper body mechanics and exercise techniques, ensuring they are well-equipped to continue their recovery process at home.
• Administer and monitor treatment modalities, including electrical stimulation, ultrasound, and heat/cold therapy, while adhering to best practice protocols.
• Document patient progress meticulously, tracking key metrics such as range of motion, strength, and functional improvements to adjust treatment plans as necessary.
• Prepare treatment areas and ensure that therapy equipment and supplies are organized and available, maintaining a safe and efficient work environment.
• Collaborate with a multidisciplinary team to ensure comprehensive care coordination, and participate in case discussions to optimize patient outcomes.
• Provide emotional support and motivation to patients, fostering a positive therapy experience that encourages progress and resilience.
• Uphold ethical and professional standards, maintaining patient confidentiality and advocating for patients' rights.

Patient Population / Client Focus

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Active physical therapy license in the state of New York.
• Minimum of one year of experience in a clinical setting, preferably within home care services.
