Work Environment

This role is primarily onsite, providing therapy interventions in patients' homes. You will be a vital part of our interdisciplinary team, working collaboratively in the community settings to ensure patients receive comprehensive care tailored to their individual needs. Key Responsibilities

• Develop and implement personalized physical therapy treatment plans focused on preventing disabilities, alleviating pain, enhancing muscle function, and improving the overall performance of clients.
• Supervise and guide Physical Therapy Assistants to ensure adherence to treatment protocols and the effective delivery of therapy services.
• Conduct thorough evaluations of patients' physical abilities using advanced diagnostic and prognostic assessments.
• Analyze the home environment of clients, making recommendations to support their rehabilitation and independence.
• Administer a variety of therapeutic interventions, including manual exercises, functional activities, and treatments involving specialized physical agents like traction and massage.
• Evaluate, fit, and adjust orthopedic devices, providing recommendations for modifications to enhance patient mobility.
• Document all aspects of patient care meticulously, generating clinical records that include evaluations, care plans, progress notes, and discharge summaries according to OASIS requirements.
• Collaborate with the interdisciplinary team, actively participating in case conferences to assess and modify treatment plans continually.
• Educate patients and families on therapy devices and self-management strategies to foster compliance and empowerment in their health journey.
• Engage with community resources to support client care, completing functional assessments during home visits to ensure optimal service delivery.
• Maintain timely submission of all documentation relevant to clinical care and payroll as per organizational policies.

The ideal candidate will work closely with a diverse patient population, primarily focusing on individuals recovering from orthopedic surgeries, injuries, or chronic conditions that impact mobility and functional independence. Qualifications

• Minimum of a Doctorate in Physical Therapy (DPT) from an accredited program.
• Valid state license to practice as a Physical Therapist in New York.

• Proven ability to make informed decisions under pressure and take on full responsibility for patient care.
• At least one year of experience in physical therapy, particularly in acute care or home care settings, is preferred.
• Strong interpersonal and communication skills to engage effectively with clients and their families.

Preferred Experience

• Experience with home care service delivery and an understanding of community resources for patient support.
• Familiarity with electronic medical record (EMR) systems to enhance documentation efficiency.
• Background in clinical education, supervision of assistants, or mentorship roles.
