Key Responsibilities\n

As a Licensed Professional Counselor at Molina Healthcare & Wellness Center, your responsibilities will include:

• Collaborating with primary care providers to identify and manage mental health and psychosocial issues within the clinic.

• Conducting thorough patient intakes, which include diagnostic assessments and functional evaluations, followed by formulating treatment recommendations for primary care providers.
\n
• Providing brief yet effective interventions for patients requiring mental health services while meeting established productivity targets for billable services each week.
\n
• Advising the primary care team on referrals to specialty mental health facilities when necessary, ensuring that appropriate care pathways are established.
\n
• Monitoring patient progress through regular follow-up consultations, adjusting treatment plans based on outcomes and feedback.
\n
• Developing relapse prevention strategies for patients to maintain stability in their mental health, along with closely observing at-risk patients and proactively preventing deterioration of their conditions.

• Documenting patient progress accurately, ensuring that all clinical records meet compliance standards while working collaboratively with the primary care provider on treatment plans.
\n
• Providing consultation to clinic management regarding behavioral health services, outcomes, and evaluation methods to enhance service effectiveness.

• Educating patients on self-management techniques and skills to empower them in their recovery process, fostering greater involvement in their own treatment plans and lifestyle changes.

Patient Population / Client Focus\n

Your clinical practice will focus on diverse patient populations, which may include individuals from various socioeconomic backgrounds experiencing a spectrum of mental health issues. Qualifications\n

To qualify for this role, candidates must have:

• A Master’s Degree in Clinical Counseling or a related field.

• A current LPC (Licensed Professional Counselor) license.
