Key Responsibilities\n

  Conduct thorough assessments of patients’ psychosocial and emotional needs to inform care planning and treatment.

  Provide short-term counseling and crisis intervention, offering immediate support to patients in distress.

  Guide patients through the process of completing necessary forms for entitlements, insurance, and other benefits, ensuring they receive the resources they need.

  Assist patients in accessing medication cost assistance through partnerships with pharmaceutical companies and community resources.

  Advocate on behalf of patients when liaising with community organizations and service providers to ensure comprehensive care.

  Maintain accurate and confidential patient records, preparing necessary documentation for case management and reporting.

  Coordinate transportation assistance for patients when required, ensuring access to services as needed.

Qualifications\n

  A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

  A minimum of two (2) years of professional experience in a mental health counseling role is essential.

Preferred Experience\n

Compensation and Benefits\n
