Key Responsibilities\n

• Conduct individualized assessments to develop personalized treatment plans and provide tailored individual and group counseling.

• Complete triage and crisis interventions for clients as necessary, ensuring timely support during critical moments.

• Offer case management services while collaborating with referral clients and community resources to enhance care delivery.

• Utilize various therapeutic modalities—including cognitive behavioral therapy, motivational interviewing, and relaxation training—to meet the unique needs of clients.

• Participate actively in a collaborative approach to care by attending multidisciplinary meetings, case consultations, and team coordination efforts with psychological and medical staff.

• Plan and implement outreach activities that promote wellness initiatives and connect clients to valuable community resources.

Patient Population / Client Focus\n

Your primary clients will be individuals who are undergoing detoxification and treatment for substance use disorders. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.
