Key Responsibilities\n

• Conduct comprehensive patient intakes that include diagnostic and functional evaluations, followed by the formulation of tailored treatment plans.

• Provide brief and focused mental health interventions for patients encountering psychosocial challenges, thereby supporting their journey towards recovery.
\n
• Collaborate with primary care providers to identify patients who may benefit more from specialty services, ensuring timely referrals as necessary.

• Monitor treatment progress and initiate follow-up communications with patients to assess the effectiveness of implemented strategies.
\n
• Develop and implement relapse prevention plans, equipping patients with coping strategies and support networks to maintain their well-being.

• Document all patient interactions, progress notes, and treatment plans in accordance with agency guidelines, ensuring thorough records for each patient.

• Educate patients about self-management skills and provide resources to facilitate active participation in their own treatment plans.

• Maintain open and ongoing communication with both patients and healthcare team members regarding evolving treatment needs and adjustments to care.

• Stay abreast of community resources to assist patients in connecting with external support services.

The Licensed Professional Counselor will work with a diverse population, including individuals facing a range of mental health challenges such as anxiety, depression, trauma, and life transitions. Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a related field is required.

• Current licensure as a Licensed Professional Counselor (LPC) in the state of Texas is mandatory.

Compensation and Benefits\n

The compensation for this position is competitive, ranging from USD 87,000 to 105,000 per year, commensurate with experience and qualifications.
