Work Environment

This position is an onsite role where you will be providing services directly to patients in their homes. Our facility emphasizes a supportive and resource-rich environment, designed to equip you with the tools and training necessary for optimal patient care. The scheduled hours for this position are Monday to Friday, from 9:00 AM to 5:00 PM, allowing for a balanced professional life.

Key Responsibilities

• Conduct thorough assessments for each patient, gathering a detailed medical history and identifying specific rehabilitation needs.
• Develop customized treatment plans aimed at improving strength and mobility based on initial assessments.
• Provide one-on-one physical therapy sessions, helping up to 7 patients per shift, utilizing innovative therapy methods and techniques.
• Monitor and document patient progress accurately, ensuring all records are up to date and compliant with healthcare regulations.
• Communicate effectively with patients and their families, addressing any inquiries and keeping them informed about therapy milestones.
• Work collaboratively with other healthcare professionals, including occupational therapists and doctors, to ensure a holistic approach to patient recovery.

The patients at Moving & Grooving Rehab Center are primarily adults recovering from surgery, injury, or chronic illness requiring rehabilitation. Qualifications

• An active Physical Therapist License issued by the New York State.
• A Bachelor's degree in Physical Therapy, with a Master's or Doctorate preferred.

Preferred Experience

Compensation and Benefits
