Work Environment

This role is conducted onsite at patients' homes, allowing you to provide therapy in a familiar and comfortable setting. Key Responsibilities

• Treat an average of 7 patients per day, tailoring each session to meet the individual needs of each client.
• Conduct thorough evaluations to develop personalized treatment plans based on the latest evidence-based practices.
• Utilize a variety of modalities and techniques, including manual therapy, therapeutic exercises, and education, to promote recovery.
• Provide clear, empathetic communication with patients and their families, ensuring they understand treatment goals and progress.
• Monitor patient outcomes and modify therapeutic approaches as needed to optimize recovery and rehabilitation.
• Maintain detailed and timely documentation of patient progress, ensuring compliance with regulatory standards.
• Work collaboratively with other healthcare providers, including physicians and occupational therapists, to coordinate comprehensive patient care.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, primarily consisting of adults recovering from surgery, injuries, or chronic conditions. Your ability to adapt treatment strategies to meet the physical and emotional needs of each patient will be crucial in helping them achieve their rehabilitation goals and regain mobility.

Qualifications

• Valid New York State Physical Therapist License.
• Bachelor's degree in Physical Therapy from an accredited institution; advanced degrees (MSPT or DPT) are preferred.
• At least two years of clinical experience in physical therapy, preferably in a home care or skilled nursing setting.

Preferred Experience

• Familiarity with electronic health record systems for efficient documentation and patient management.
• Experience developing and implementing customized therapy plans based on individual patient assessments.
• Strong interpersonal skills and a knack for motivating patients through their recovery processes.

Compensation and Benefits

We offer a competitive salary range of $100,000 to $120,000 annually, commensurate with experience.
