Echo Rehabilitation Center is seeking a dedicated Physical Therapist to join our team in Warwick, New York. Work Environment

This role is based in Warwick, NY, primarily focusing on delivering home care services. You will collaborate with a multidisciplinary team of healthcare professionals in the community, provided with the necessary resources and support to thrive in your role. Our work hours are Monday to Friday, from 9:00 am to 5:00 pm, allowing for a balanced work-life schedule.

Key Responsibilities

• Assess and evaluate up to seven patients per day in their home environments, ensuring individual needs are met.
• Develop comprehensive, evidence-based treatment plans tailored specifically to enhance mobility, strength, and overall function.
• Utilize a variety of therapeutic techniques, including manual therapy, therapeutic exercises, and functional training, to support recovery goals.
• Continuously monitor and document patient progress, adjusting plans as necessary to maximize therapeutic outcomes.
• Educate patients and their families on rehabilitation exercises, strategies for daily living, and preventative measures to maintain health improvements.
• Collaborate effectively with other healthcare professionals, such as occupational therapists, nurses, and social workers, to coordinate patient-centered care.

Patient Population / Client Focus

The Physical Therapist will work primarily with a diverse patient population, including seniors recovering from surgery, individuals with chronic conditions, and patients rehabilitating from injuries. Your expertise will directly impact their functional independence and quality of life, making your role crucial in our home care delivery model.

Qualifications

• Valid New York State Physical Therapist License.
• Minimum of a Bachelor's Degree in Physical Therapy; an MSPT or DPT is preferred.
• At least two years of clinical experience in a rehabilitation or home care setting.

Preferred Experience

While a minimum of two years of experience is required, candidates with additional experience in geriatric care, neurological rehabilitation, or orthopedic therapy will be given preference. Familiarity with electronic medical records (EMR) systems will be advantageous for efficient documentation and care coordination.

Compensation and Benefits

This position offers a competitive salary ranging from $100,000 to $120,000 annually, commensurate with experience, and includes a comprehensive benefits package. Benefits provided include:
