Work Environment

This position is onsite and involves traveling to patients' residences within the Goshen area. Key Responsibilities

• Provide high-quality physical therapy services to patients within a home care setting, seeing an average of 7 patients per day.
• Conduct thorough assessments of patients' physical abilities and needs to create individualized treatment plans that address their unique challenges.
• Employ various therapeutic modalities, including manual therapy, therapeutic exercises, and home exercise programs, to promote functional gains and improve patients' quality of life.
• Collaborate with other healthcare professionals, such as occupational therapists and physicians, to monitor patient progress and adjust treatment plans as necessary.
• Document all patient interactions, assessments, and progress notes accurately and timely within our electronic health record (EHR) system to ensure continuity of care.
• Educate patients and their families about rehabilitation processes, home safety, and prevention strategies to promote long-term health.

Patient Population / Client Focus

The Physical Therapist will primarily work with a diverse patient population, including elderly clients recovering from surgery, individuals with chronic illness, and patients with neurological or orthopedic conditions. Compassionate communication and a tailored approach to therapy are essential for addressing the varying needs and goals of each patient in their home environment.

Qualifications

• Valid New York State Physical Therapist License.
• Bachelor's degree in Physical Therapy or an equivalent degree from an accredited institution.
• A minimum of two years of experience in a rehabilitation or physical therapy setting, preferably in home care.

Preferred Experience

Experience in a skilled nursing facility or home health care is advantageous, as is familiarity with rehabilitation techniques for both acute and chronic conditions. Candidates who have a background in developing home exercise programs or working with geriatric populations will be given priority.

Compensation and Benefits
