Key Responsibilities

• Conduct comprehensive assessments of patients' physical capabilities, identifying specific limitations and areas for improvement to formulate individualized treatment plans.
• Utilize evidence-based hands-on therapy techniques and therapeutic exercises tailored to each patient's needs, focusing on improving strength, balance, and functional mobility.
• Regularly monitor and document patient progress, adjusting treatment strategies as necessary to maximize rehabilitation outcomes.
• Provide thorough education to patients and their caregivers about rehabilitation exercises and self-management techniques to facilitate ongoing recovery outside of therapy sessions.
• Maintain accurate and detailed documentation to ensure continuity of care and facilitate communication with the interdisciplinary care team.
• Engage in collaborative discussions with team members, contributing to a unified care plan and fostering an environment of shared knowledge and expertise.

Patient Population / Client Focus

Your clinical expertise will primarily serve a diverse patient population comprising individuals of all ages recovering from post-surgical procedures, traumatic injuries, and chronic ailments. Qualifications

• Possession of a valid and active Physical Therapist license in the state of New York.
• Completion of a Master's degree or Doctorate in Physical Therapy from an accredited program.
• Minimum of two years of clinical experience, particularly in skilled nursing or rehabilitation settings, is required.
