Work Environment

This role is primarily conducted in patients' homes, necessitating a compassionate and adaptable approach. The onsite nature of this position allows you to build meaningful relationships with patients in their own environments, where care feels more personal and relevant. You will work closely with a multidisciplinary care team, ensuring that every aspect of the patient's rehabilitation journey is addressed with holistic care and support.

Key Responsibilities

• Deliver tailored therapy services to an average of 7 clients per day, focusing on individual recovery goals.
• Develop and implement comprehensive treatment plans, assessing patient progress and modifying approaches as needed.
• Conduct mobility and rehabilitation exercises, instructing patients on proper techniques to foster safety and effectiveness.
• Utilize advanced equipment and therapeutic modalities, such as ultrasound and electrical stimulation, to enhance treatment efficacy.
• Maintain meticulous documentation of patient care and progress, ensuring secure and organized patient records in compliance with regulatory standards.
• Communicate effectively with patients and their families, providing education and encouragement to facilitate their understanding of the recovery process.
• Collaborate seamlessly with healthcare professionals, including nurses and occupational therapists, to coordinate comprehensive patient care.

Patient Population / Client Focus

In this role, you will cater to a diverse patient population, primarily focusing on older adults experiencing mobility issues due to post-surgical recovery, chronic conditions, or neurological impairments. Your role is crucial in assisting patients to regain independence in their daily activities and enhancing their overall well-being. The support you provide will empower them to overcome challenges and achieve their rehabilitation goals.

Qualifications

• A valid New York State Physical Therapist License.
• A Bachelor's degree in Physical Therapy, with preference given to candidates holding a Master's or Doctorate in Physical Therapy.
• At least 2 years of clinical experience, particularly in rehabilitative or home care settings.

Preferred Experience

• Experience with geriatrics and knowledge of age-related mobility challenges.
• Proficiency in using therapeutic equipment commonly utilized in home care settings.
• Prior experience working with a multidisciplinary team to manage complex patient cases.

Compensation and Benefits

This position offers a competitive annual salary ranging from $100,000 to $120,000 , commensurate with experience and qualifications. Full-time employees are also entitled to comprehensive health benefits, a 401(k) plan with employer contributions, and a structured work schedule from Monday to Friday, 9:00 AM to 5:00 PM, ensuring work-life balance.
