Echo Rehabilitation Center is actively seeking a dedicated and licensed Physical Therapist to join our home care team in Monroe, New York. Work Environment

This position is based onsite as part of our home care facility, allowing our physical therapists to work closely with patients in their own environments. You will collaborate with a multidisciplinary team, including occupational therapists and nursing staff, to ensure a holistic approach to patient care. The work hours are Monday to Friday, from 9:00 am to 5:00 pm, providing a consistent schedule that complements work-life balance.

Key Responsibilities

• Evaluate and assess patients' physical conditions through comprehensive assessments and diagnostic tests.
• Develop customized treatment plans that address the specific needs and goals of each patient, ensuring alignment with their recovery objectives.
• Implement various therapeutic interventions, including manual therapy techniques, therapeutic exercises, and mobility enhancement strategies.
• Monitor patients' progress by recording their improvements and setbacks, adjusting treatment plans as necessary to optimize outcomes.
• Educate patients and their families on injury prevention, pain management, and effective home exercise programs.
• Coordinate with other team members to conduct interdisciplinary meetings for comprehensive case reviews.

Patient Population / Client Focus

The ideal candidate will have experience handling a diverse client base, including elderly individuals recovering from surgeries, patients with chronic pain, and those rehabilitating from neurological conditions. Our role is pivotal in enabling patients to regain independence and improve their quality of life through tailored rehabilitation solutions.

Qualifications

• A valid New York State Physical Therapist License is required.
• A Bachelor's Degree in Physical Therapy or an equivalent field is mandatory; advanced degrees such as a Master of Science in Physical Therapy (MSPT) or Doctor of Physical Therapy (DPT) are preferred.
• A minimum of two years of clinical experience is required, showcasing a competence in various therapeutic practices.

Preferred Experience

• Experience working in home care settings or with geriatric populations is highly regarded.
• Familiarity with electronic health record (EHR) systems is advantageous for documenting patient progress efficiently.
• Proficiency in applying evidence-based therapy techniques and modalities that support optimal patient recovery.

Compensation and Benefits

Offered compensation includes a competitive salary ranging from $100,000 to $120,000 annually, commensurate with experience.
