Work Environment

This position is full-time and is conducted onsite within our state-of-the-art facility. Key Responsibilities

• Conduct comprehensive evaluations and assessments of patients' physical conditions, identifying functional limitations and impairment.
• Create and implement individualized treatment plans designed to promote recovery, enhance mobility, and improve the overall physical functioning of each resident.
• Administer a variety of therapeutic exercises, manual therapy techniques, and modalities tailored to alleviate pain and treat physical impairments encountered by patients.
• Regularly monitor and document patients' progress, making adjustments to treatment plans based on observed outcomes and patient feedback.
• Collaborate effectively with interdisciplinary teams, including physicians and nursing staff, to establish comprehensive care pathways that optimize patient outcomes.
• Educate patients and their families on home exercise programs and self-care strategies, empowering them to maintain their progress outside of therapy sessions.
• Ensure compliance with all facility protocols and state regulations while adhering to best practices in physical therapy.

Patient Population / Client Focus

Our residents primarily include individuals recovering from surgeries, strokes, or those requiring rehabilitation following injuries or chronic illness. You will engage with a diverse patient population that values compassion and personalized care. Your role is crucial in helping our residents achieve their rehabilitation goals and improve their day-to-day quality of life.

Qualifications

• Education: Bachelor's or Master's degree from an accredited Physical Therapy program; advanced qualifications such as a Doctor of Physical Therapy (DPT) are advantageous.
• Licensure: Must hold a current and unrestricted license to practice Physical Therapy in New York State.
• Strong clinical skills to effectively assess and determine the best rehabilitation strategies for individual patient circumstances.
• Excellent communication and interpersonal skills to foster a positive healing environment for patients and families.
• A compassionate approach and commitment to patient-centered care.

Preferred Experience

• A minimum of 2 years of experience in a skilled nursing facility, rehabilitation center, or related setting is preferred.
• Experience with electronic documentation systems and familiarity with therapeutic modalities specific to physical rehabilitation.
• Demonstrated ability to work collaboratively in a team-based environment with a focus on achieving shared goals.
