Work Environment

This role is based onsite within our well-established skilled nursing facility. The physical therapist will collaborate closely with an interdisciplinary team consisting of physicians, nurses, and other rehabilitation specialists. Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions through assessments that focus on mobility, strength, flexibility, and functional limitations.
• Create and implement personalized treatment plans tailored to the specific needs and goals of each resident, optimizing their pathway to recovery.
• Utilize a variety of therapeutic techniques including therapeutic exercises, manual therapy, and advanced modalities to alleviate pain and restore function.
• Regularly monitor and document patient progress, adjusting treatment plans and interventions based on real-time feedback and measurable outcomes.
• Engage in collaborative care by participating in interdisciplinary team meetings to coordinate care tasks and ensure comprehensive treatment approaches.
• Educate patients and their families on effective home exercise programs as well as strategies that facilitate ongoing recovery outside of the facility.
• Adhere to all facility protocols and state regulations, maintaining the highest standards of care and compliance in all therapeutic interventions.

Patient Population / Client Focus

The physical therapist will primarily serve older adults residing in our skilled nursing facility, many of whom face complex healthcare challenges including post-surgical recovery, chronic pain management, and mobility impairments. These residents may require focused rehabilitation following surgeries such as hip or knee replacements, strokes, or other debilitating medical conditions. Providing compassionate care that recognizes each patient's unique circumstances is crucial in helping them regain independence and confidence.

Qualifications

• A valid and active Physical Therapy license in the state of New York is mandatory.
• A Bachelor's or Master's degree in Physical Therapy from an accredited institution is required; Doctor of Physical Therapy (DPT) preferred.
• Exceptional clinical skills combined with effective communication and interpersonal abilities.
• A strong commitment to patient-centered care with an emphasis on empathy and respect for the individuals we serve.

Preferred Experience

• A minimum of 2 years of experience in a similar setting, preferably within skilled nursing or rehabilitation centers.
• Experience in evidence-based practice and familiarity with integrated care models.
• Background in restorative care and rehabilitation techniques that enhance patient engagement in therapy.

Compensation and Benefits

We offer a competitive salary in the range of $100,000 to $120,000, commensurate with experience, along with a comprehensive benefits package that includes:
