Work Environment

This position operates onsite within the patients' homes, providing a unique and personal experience for the patients and their families. As a Physical Therapist at Echo Rehabilitation Center, you will engage directly with patients in their own living environments, allowing for the consideration of their daily routines and lifestyle in therapy planning. Working hours are scheduled from Monday through Friday, 9:00 AM to 5:00 PM, allowing for a healthy work-life balance.

Key Responsibilities

• Conduct comprehensive initial evaluations, including physical assessments and functional status analysis, to determine patient needs and establish baseline measurements.
• Develop and implement individualized treatment plans based on assessments, incorporating evidence-based practices tailored to each patient's unique restrictions, goals, and circumstances.
• Utilize a wide range of therapeutic techniques and modalities, such as manual therapy, therapeutic exercise, and neuromuscular re-education, to promote mobility, strength, and endurance.
• Monitor and document patient progress through regular re-evaluations and updates to the treatment plan, ensuring measurable improvements in function and quality of life.
• Educate patients and their families about home exercises, resulting in greater engagement and adherence to therapy regimens.
• Collaborate with a multi-disciplinary healthcare team, including physicians, nurses, and occupational therapists, to provide integrated, holistic care to patients.

Patient Population / Client Focus

The Physical Therapist will work primarily with patients who are recovering from surgeries, strokes, orthopedic injuries, and chronic pain conditions within their homes. Qualifications

• Current New York State Physical Therapist License is mandatory.
