Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities, limitations, and goals.
• Develop and implement tailored treatment plans, adapting them based on ongoing assessments and patient progress.
• Manage a daily caseload of up to 7 patients, ensuring each receives comprehensive and timely care throughout their treatment.
• Educate patients and their families about treatment procedures and expected outcomes, providing clear and compassionate answers to their inquiries.
• Utilize advanced rehabilitation tools and exercises, along with therapeutic modalities, to enhance recovery and mobility.
• Maintain meticulous documentation of each patient's progress and treatment plans, adhering to regulatory standards and organizational policies.
• Participate in interdisciplinary team meetings to discuss patient care strategies and share insights to foster improved patient outcomes.
• Monitor patients' progress and make timely adjustments to treatment strategies to ensure optimal recovery goals are met.

Patient Population / Client Focus

The Physical Therapist will work primarily with adult and elderly patients who may have undergone surgery, experienced trauma, or suffer from chronic health conditions affecting their physical mobility. Qualifications

• A valid New York State Physical Therapist license is required.
• A Bachelor's Degree in Physical Therapy or equivalent academic credential.

• Two or more years of relevant work experience in physical therapy, preferably within a home care or rehabilitation setting.

Compensation and Benefits
