Work Environment

This position is primarily onsite in a home care setting, allowing therapists to provide care where patients feel most comfortable: their own homes. Key Responsibilities

• Perform comprehensive assessments to identify physical therapy needs and develop individualized, evidence-based rehabilitation plans that align with patient goals.
• Implement and execute therapeutic interventions, including manual therapy techniques, movement-based treatment, and functional strength training exercises tailored to each patient's abilities.
• Educate patients and their families on injury prevention, optimal recovery strategies, and self-management techniques to empower them in their healing journey.
• Regularly evaluate patient progress and modify treatment plans as necessary to ensure that therapeutic goals are met effectively.
• Document all patient interactions and treatment plans thoroughly, adhering to best practices for patient confidentiality and compliance.

Patient Population / Client Focus

The ideal candidate will be working primarily with individuals recovering from orthopedic surgeries, sports injuries, and chronic musculoskeletal conditions. Our patient base may include athletes seeking to return to their sport, seniors looking to regain mobility, and other adults requiring specialized rehabilitation services. The ability to build rapport and motivate patients through their recovery journey is essential in this role.

Qualifications

• Possession of an active Physical Therapist license in the state of New York.
• A Bachelor's degree in Physical Therapy; preferred Master's or Doctorate in Physical Therapy.
• Strong communication and interpersonal skills to effectively interact with patients, families, and multidisciplinary team members.
• Demonstrated commitment to ongoing professional development and education.

Preferred Experience

• A minimum of two years' experience in physical therapy, particularly within sports medicine or outpatient orthopedic settings.
• Experience implementing comprehensive rehabilitation strategies and a history of successful patient outcomes.
• Familiarity with electronic health records (EHR) systems is a plus.

Compensation and Benefits

We offer a competitive salary range of $100,000 to $120,000, commensurate with experience and qualifications.
