Work Environment

This position is primarily onsite, focusing on home care, which allows therapists to work one-on-one with patients in the comfort of their own residences. The structured work schedule runs from Monday to Friday, 9 AM to 5 PM, providing consistency and stability in your work life.

Key Responsibilities

• Perform comprehensive assessments of patients' physical conditions, identifying musculoskeletal issues and functional limitations through in-depth evaluations.
• Design and implement customized rehabilitation programs that include manual therapy techniques, therapeutic exercises, and functional training to enhance mobility, strength, and activity performance.
• Instruct patients and their families on recovery techniques, strategies for injury prevention, and advice on maintaining their wellness after therapy sessions conclude.
• Regularly collaborate with a multidisciplinary team of healthcare professionals, including physicians, occupational therapists, and nurses, to coordinate care and ensure an integrated approach towards patient rehabilitation.
• Document and maintain thorough patient records and progress notes, utilizing our electronic health record (EHR) system to ensure accurate and timely treatment tracking.

Patient Population / Client Focus

Your primary focus will be working with diverse patient populations ranging from post-operative rehabilitation clients to those with chronic pain or mobility issues. Engaging with patients of various ages and backgrounds requires adaptability and a deep understanding of personalized therapeutic techniques. Whether a patient is recovering from surgery, managing a chronic condition, or seeking to improve their daily functional abilities, the goal is to support their journey to recovery and independence.

Qualifications

• A valid New York State Physical Therapist license is mandatory.
• Possession of a Bachelor's degree in Physical Therapy from an accredited institution; advanced degrees such as a Master's or Doctorate are advantageous.

Preferred Experience

• While outpatient experience is advantageous, we encourage recent graduates who are eager to learn and grow in a supportive environment to apply.
• A background in home health care is a plus, but not a requirement.

Compensation and Benefits
