Work Environment

This position is designed for an onsite Physical Therapist who will engage with patients in their homes, providing a comfortable and familiar setting for therapy. Key Responsibilities

• Evaluate patients' physical conditions by conducting comprehensive assessments, including strength and mobility evaluations, to develop tailored treatment plans.
• Administer therapies such as manual therapy, therapeutic exercises, and modalities to assist in patient rehabilitation, ensuring adherence to safety and clinical protocols.
• Meet with an average of seven patients daily, providing hands-on treatment and monitoring their progression toward individualized goals.
• Maintain clear and timely communication with patients and their families, addressing their concerns and guiding them through their rehabilitation journey.
• Document treatment sessions accurately in electronic health records (EHR), ensuring compliance with organizational standards and insurance requirements.
• Collaborate with interdisciplinary team members, including occupational therapists and physicians, to optimize patient care strategies.
• Provide education on therapeutic techniques and home exercise programs, empowering patients to take an active role in their recovery.

Patient Population / Client Focus

The patients you will work with range from post-operative individuals to those managing chronic conditions affecting mobility and functionality. Your role will be pivotal in addressing a variety of needs, including orthopedic rehabilitation, geriatric care, and recovery from neurological impairments. Understanding the diverse backgrounds and conditions of your patients will allow you to create effective and empathetic treatment plans.

Qualifications

• Valid New York State Physical Therapist License.
• Bachelor's Degree in Physical Therapy required; a Master's (MSPT) or Doctorate (DPT) preferred.
• A minimum of two years of relevant experience in a clinical or home care setting.

• Experience working with diverse patient populations, particularly in home care settings.
• Knowledge of therapeutic modalities and advanced rehabilitation techniques.

Compensation and Benefits
