Work Environment

This role is primarily onsite, allowing for direct interaction with patients in the comfort of their homes. The work schedule is Monday to Friday from 9:00 AM to 5:00 PM, promoting a healthy work-life balance while ensuring consistent patient engagement.

Key Responsibilities

• Conduct thorough assessments of patients' physical conditions to identify limitations and establish personalized treatment plans aimed at enhancing mobility and function.
• Utilize hands-on therapy techniques, including manual therapy, therapeutic exercises, and modalities, to promote recovery and improve strength, balance, and coordination.
• Continuously monitor patient progress, dynamically adjusting therapy plans where necessary to optimize outcomes and provide motivation throughout their recovery journey.
• Educate patients and their families on rehabilitation exercises and techniques to foster continued progress and independence outside of therapy sessions.
• Maintain meticulous and timely documentation of patient evaluations, treatments, and progress to ensure accountability and effective communication with interdisciplinary team members.
• Collaborate with a range of healthcare professionals including nurses, physicians, and occupational therapists to develop comprehensive care plans that address all aspects of patient well-being.

Patient Population / Client Focus

This role engages with a diverse patient population, including individuals recovering from orthopedic surgeries, trauma, stroke, chronic pain, and other conditions that impact physical mobility and functional independence. Our therapists are committed to providing empathetic care that not only addresses physical rehabilitation but also supports emotional and psychological well-being during the healing process.

Qualifications

• Valid Physical Therapist License issued by the New York State Board.
• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
• Two or more years of clinical experience, ideally in home care or skilled nursing settings, where you developed tailored treatment strategies that align with patient needs.
• Exceptional communication skills with a strong emphasis on a patient-centered approach and the ability to motivate and encourage patients throughout their therapy journey.

Preferred Experience

• Experience using electronic health record (EHR) software for patient documentation and progress tracking.
• Specialized training in manual therapy techniques or rehabilitation for specific patient populations, such as geriatrics, sports rehab, or post-operative care.
• A background in interdisciplinary team collaboration and experience working alongside other healthcare professionals to provide holistic care.
