Work Environment

This position is onsite as part of our home care team, where you will visit patients in their homes to provide direct therapeutic services. Work hours are from Monday to Friday, 9:00 am to 5:00 pm, allowing for a balanced work-life dynamic. Key Responsibilities

• Conduct thorough evaluations of each patient by assessing their physical function, mobility, and pain levels to create individualized, evidence-based treatment plans.
• Administer a range of therapeutic interventions, including manual therapy techniques and exercise rehabilitation tailored to the patient's specific needs and goals.
• Set measurable goals for patient progress and regularly monitor outcomes, making adjustments to treatment plans as necessary to ensure optimal recovery.
• Educate patients and their families on helpful exercises and lifestyle modifications that support ongoing recovery and overall well-being.
• Collaborate with interdisciplinary teams, including physicians and occupational therapists, to develop comprehensive care plans that address all aspects of a patient's health.
• Document patient progress accurately in accordance with state regulations and organizational policies, ensuring clear communication among team members.
• Address any barriers to patients' rehabilitation, proposing innovative solutions to enhance their engagement in the therapeutic process.

Patient Population / Client Focus

The patient population we serve primarily includes geriatric clients, patients recovering from surgery, and individuals with chronic conditions or post-acute care needs. Qualifications

• New York State Physical Therapist License is required.
• A Bachelor's degree in Physical Therapy or a related field is mandatory.
• A minimum of two years of experience as a Physical Therapist, preferably in a home care or rehabilitation setting.

• Experience with geriatric populations and knowledge of age-related conditions.

Compensation and Benefits
