Position Overview

Work It PT is searching for a dedicated and skilled Physical Therapist (PT) to join our team at our state-of-the-art Skilled Nursing Facility located in Monroe, New York. The individual in this full-time position will be responsible for delivering high-caliber rehabilitative care tailored to meet the unique needs of our residents. Key Responsibilities

• Conduct comprehensive evaluations of residents to assess their physical conditions, limitations, and rehabilitation goals, utilizing advanced diagnostic tools and methodologies.
• Design and implement individualized treatment plans that leverage therapeutic exercises, manual therapy techniques, and a variety of modalities (e.g., electrical stimulation, ultrasound) to promote recovery.
• Regularly monitor and document patient progress, making data-driven adjustments to treatment plans to ensure optimal outcomes.
• Collaborate with a multidisciplinary team composed of physicians, nurses, occupational therapists, and speech therapists to coordinate comprehensive care.
• Educate patients and their families on home exercise programs and best practices for ongoing care, empowering them to take an active role in their rehabilitation journey.
• Adhere to the facility's regulations and best practices, maintaining compliance with state and federal guidelines at all times.
• Participate in regular team meetings to discuss patient progress, identify barriers to rehabilitation, and share insights that drive continuous improvement in care quality.

Patient Population / Client Focus

The Physical Therapist will primarily serve residents recovering from surgeries, injuries, or chronic illnesses, including but not limited to post-operative rehabilitation, neurorehabilitation, and supportive care for individuals with degenerative conditions. By individualizing care plans based on the unique needs and progression of each patient, the therapist will strive for improved outcomes in mobility and quality of life.

Qualifications

• A Doctor of Physical Therapy (DPT) degree or a Bachelor's or Master's in Physical Therapy from an accredited institution.
• An active and unrestricted Physical Therapy license in the state of New York.
• Strong clinical competencies, including familiarity with the latest practice guidelines and rehabilitation techniques.
• Excellent communication and interpersonal abilities, demonstrating empathy and respect to all patients and their families.
• A patient-centered approach that prioritizes the dignity and preferences of residents while promoting their recovery.

Preferred Experience

• At least 2 years of experience in a skilled nursing facility, rehabilitation center, or similar setting is preferred.
• Previous experience working with geriatric populations and understanding of age-related physical conditions.
• Exposure to telehealth platforms and flexibility to incorporate technology into therapy sessions.
