Key Responsibilities

• Evaluate and administer effective physical therapy treatments to a diverse caseload, averaging seven patients per day, showcasing adaptability to various rehabilitation needs and conditions.
• Develop personalized treatment plans based on thorough assessments and communicate these plans clearly to both patients and their families, ensuring full understanding and cooperation on the path to recovery.
• Guide patients in the execution of treatment plans, providing hands-on therapy and education while encouraging proper home exercise and wellness strategies.
• Utilize electronic health record (EHR) systems to efficiently document patient progress, maintaining accurate records that ensure ongoing communication with the care team and comply with regulatory standards.
• Conduct regular reviews of patient outcomes, modifying treatment plans based on patient progress and feedback to optimize recovery trajectories.
• Engage actively in team meetings and training sessions to share insights and enhance the collective knowledge and skills of the rehabilitation staff.

Patient Population / Client Focus

At Echo Rehabilitation Center, you will work primarily with geriatric patients who experience a range of health challenges, including post-surgical rehabilitation, neurodegenerative diseases, and musculoskeletal injuries. Your expertise will be crucial in guiding these patients towards regaining functionality and independence.

Qualifications

• Valid New York State Physical Therapist License.
• Bachelor's Degree in Physical Therapy (MSPT or DPT preferred).
• A minimum of two years of relevant clinical experience, preferably within a skilled nursing or rehabilitation setting.

Preferred Experience

• Previous experience in a skilled nursing facility or within a similar rehabilitative environment.

Compensation and Benefits
