Key Responsibilities\n

• Conduct comprehensive assessments and develop individualized treatment plans tailored to the unique needs of each patient, including pre-crisis and crisis plans, behavior guidelines, and medication treatment strategies.

• Complete all necessary documentation promptly and in accordance with regulatory standards, ensuring compliance with state and federal guidelines for telehealth services.

• Utilize evidence-based screening tools as recommended by the clinical team to inform treatment decisions and assess patient progress.

• Collaborate with interdisciplinary teams to create and implement positive behavior support plans that aim to enhance patient outcomes.
\n
• Participate actively in licensing and accreditation processes, maintaining awareness of best practices within the mental health field.
\n
• Act as a liaison with relevant service systems, including state agencies and community partners, to coordinate care and connect patients with additional resources.

• Establish and foster open lines of communication with stakeholders to meet contract standards and align with the mission of Elevated Healing Place Healthcare in the broader community.

• Engage in continuous professional development through conferences, workshops, and by participating in statewide meetings related to mental health services.

• Promote and embody the agency’s mission, values, and principles in all interactions with patients and colleagues.

• Execute additional duties as assigned by the Clinic Director to support the clinic's operations and objectives.

The patients you will serve encompass a diverse demographic, including individuals facing mental health challenges, those in recovery from substance abuse, and individuals with developmental disabilities. Qualifications\n

• A Master’s Degree in a mental health-related field is a mandatory requirement for candidacy.

• Possession of an Independent LMHC License is essential prior to commencement of work.
