Key Responsibilities\n

• Conduct comprehensive assessments of clients to develop tailored treatment plans that address their unique needs across academic, professional, and personal dimensions.

• Provide ongoing therapeutic interventions and support through virtual consultations, facilitating progress toward clients' goals in a supportive environment.

• Monitor medication adherence by providing clients with timely prompts and assistance in managing their prescriptions responsibly, while also educating them about proper storage and efficacy.

• Assist clients with transportation needs, whether it be guiding them through public transport systems or providing personal transport when necessary, ensuring they can maintain essential medical appointments.

• Facilitate life skills development by teaching clients tasks such as meal planning, cooking, and managing daily responsibilities to foster independence and self-efficacy.

• Document client interactions meticulously, maintaining compliance with legal and regulatory requirements. This includes timely updates to electronic health records and progress notes.

• Work closely with interdisciplinary teams to ensure a coordinated approach to client care, sharing insights and strategies that contribute to holistic treatment plans.

Qualifications\n

• Self-directed with excellent organizational skills, capable of working independently without direct supervision.

Preferred Experience\n

Compensation and Benefits\n
