Work Environment

Key Responsibilities

• Conduct comprehensive evaluations to assess clients’ psychological and emotional needs.
• Develop and implement individualized care plans tailored to each client’s specific situation, goals, and preferences.
• Utilize evidence-based therapeutic methods, such as Cognitive Behavioral Therapy (CBT) and Trauma-Informed Care, to facilitate meaningful client progress.
• Maintain meticulous documentation of client interactions and treatment plans in compliance with state regulations and organizational standards.
• Collaborate with a multidisciplinary team of healthcare professionals, including physicians, nurses, and support staff, to deliver integrated patient care.
• Educate clients and their families about available resources, coping strategies, and care options to empower them in their journey.
• Engage in continuous professional development and integrate innovative approaches into practice to enhance client care.

Patient Population / Client Focus

The LCSW will primarily focus on providing services to individuals receiving hospice care, addressing their psychological and emotional needs during a challenging period in their lives. The clientele includes a diverse demographic, encompassing various cultural, socioeconomic, and age backgrounds, ensuring that every client feels valued and understood.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in California is required.
• Excellent communication and interpersonal skills to build rapport with clients and collaborate effectively with other healthcare professionals.
• A strong understanding of the unique challenges faced by hospice patients and their families.

Preferred Experience

• At least two years of clinical experience in a mental health setting, ideally with a focus on hospice or palliative care.

Compensation and Benefits
