This role is designed for remote work, allowing flexibility while you take part in fulfilling interactions with clients and team members daily. Key Responsibilities\n

• Collaborate with doctors, nurses, and other health professionals to deliver coordinated, high-quality patient care tailored specifically to the individual needs of each client.

• Conduct thorough assessments of patients’ psychosocial needs, identifying emotional, social, and environmental factors affecting their health and well-being.
\n
• Develop, implement, and monitor comprehensive care plans that address the unique challenges faced by patients and their families, adjusting plans as necessary based on patient progress.

• Provide counseling and support to patients and their family members, helping them cope with the emotional burden of illness and loss.
\n
• Stay current with best practices and innovative strategies in mental health and social work, attending regular training and professional development workshops.

Patient Population / Client Focus\n

In this role, you will provide critical support to individuals navigating the complex emotional landscapes associated with terminal illnesses. Your work will focus on alleviating psychological distress in patients who are experiencing end-of-life transitions while also offering guidance and education to their families to foster a deeper understanding of the care process.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in New Jersey.

• Strong background in conducting clinical evaluations and creating effective care plans based on client needs.

• Exceptional communication skills, with the ability to engage and collaborate effectively with diverse teams and clients.

Preferred Experience\n

• Minimum of two years of experience in social work, particularly in health care settings or directly with hospice patients.

Compensation and Benefits\n
