This position is entirely remote, providing the flexibility to work from home while remaining connected with our Hospice Center's multidisciplinary team. Key Responsibilities\n

• Conduct thorough assessments of clients' counseling needs, utilizing DSM (Diagnostic and Statistical Manual) criteria to identify substance abuse diagnoses and determine the appropriate level of care based on ASAM (American Society of Addiction Medicine) recommendations.

• Develop and create comprehensive treatment plans that include substance abuse counseling components tailored to each client's unique circumstances and needs.
\n
• Maintain an accurate clinical record by documenting all counseling sessions, assessments, and follow-up actions.
\n
• Coordinate essential support services for clients and their families, ensuring all interventions and resources are recorded appropriately in clinical documentation.

• Manage a diverse caseload effectively, conducting ongoing treatment planning and clinical charting to support positive outcomes.
\n
• Engage with referral sources, such as legal, educational, and employment representatives, to foster holistic support for clients.
\n
• Facilitate crisis interventions using a multi-disciplinary team approach to address urgent client needs promptly.
\n
• Promote an environment conducive to long-term recovery, working closely with the Clinical Director and Executive Director to maintain a therapeutic routine for residents.

Patient Population / Client Focus\n

You will work with a diverse patient population facing substance use issues and co-occurring disorders, particularly those in a hospice care setting. This role is essential in supporting patients and families during critical transitions, ensuring they have the emotional, psychological, and practical resources needed to cope with their circumstances.

Qualifications\n

• Holder of a valid LCSW license in the state of New Jersey.

• A Bachelor’s degree in Social Work or a related field.
\n
• Exceptional professionalism, strong therapeutic boundaries, and cultural competency.

• Demonstrated self-motivation, organizational skills, and the ability to manage time efficiently.

• Experience in substance use counseling and understanding of the treatment and recovery process.

Preferred Experience\n

• Previous experience working with hospice patients or individuals facing end-of-life issues.

Compensation and Benefits\n
