Key Responsibilities\n

• Utilize electronic medical records (EMR) to monitor, document, and communicate patient progress, constantly updating the social work plan to reflect your clients' evolving needs.

• Provide support and information to patients and families regarding transition plans, ensuring smooth navigation through healthcare systems.

• Maintain up-to-date knowledge of government and private payer networks to facilitate appropriate transitions in care for clients.

• Actively collaborate with community resources and healthcare organizations to coordinate comprehensive care for patients.

• Advocate for patient rights, working diligently to ensure patients access protective services when necessary.

• Demonstrate ethical decision-making in patient advocacy, ensuring compliance with legal requirements regarding the reporting of vulnerability or suspected abuse.

• Participate in initiatives that prevent hospital readmissions through proactive patient engagement and care planning.

• Organize and facilitate transition conferences with patients and their families to ensure clear communication and expectation setting.

• Employ technology and data analysis tools to identify at-risk patients and develop intervention strategies to improve health outcomes.

• Ensure comprehensive clinical handoffs for at-risk patients, which may include making referrals to additional services.

This position offers a competitive salary ranging from USD 100,000 to 105,000 per year.
