Posting Date 07/22/2026 3461 W Broadway Ave, Robbinsdale, Minnesota, 55422-2955, United States of America DaVita is seeking an organized, self-directed Float RN to manage Home Hemodialysis (HHD) patients. You'll educate patients and caregivers in-clinic, enabling them to perform dialysis independently at home. This role blends clinical care, patient education, and case management. Key Responsibilities: Float to Home Programs in the region as needed. Float locations may include: Maple Grove, Minneapolis, Apple Valley, St. Paul, Hudson and Amery, WI Train patients and caregivers on HHD procedures and self-care Manage a caseload of HHD patients, including assessments and care planning Monitor treatment progress, conduct home visits, and provide on-call support Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes Schedule: Monday-Friday, 8-hour shifts. Must be flexible as occasional late shifts and Saturdays may be needed to support the needs of HHD patients.
