Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate each client’s mental health status and identify specific needs.

• Design and implement individualized treatment plans that incorporate goals, therapeutic modalities, and interventions tailored to each client's situation.

• Provide therapeutic services for individuals, families, and groups using a variety of evidence-based practices such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.

• Facilitate group therapy sessions, psychoeducation workshops, and support groups focused on building community and resilience.

• Work collaboratively with other health care professionals including psychiatrists, psychologists, and social workers to coordinate comprehensive care that addresses both mental and physical health needs.

• Deliver crisis intervention services, perform risk assessments, and develop safety plans as required to support client safety and well-being.

• Maintain accurate clinical records and documentation in compliance with legal and ethical standards, ensuring seamless communication within the treatment team.

Your work will primarily involve clients who are facing significant life transitions, particularly those in hospice care. Qualifications\n

• A master's degree in Social Work from an accredited institution is required.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
